What Do I Need?

In order to schedule an consultation, patients will need to provide Imaging Studies and/or reports to determine if we can treat your condition. You may be required to get new x-rays at the time of your appointment. We will also need the front and back of your insurance card. These can be deliver via, fax, email or mailed.
